Information for Patients
The Staff of John H. Hall, Jr., M.D., P.A. look forward to welcoming you
and hope that your visit with us is as pleasant and helpful as possible.
The following information should answer some common concerns you may have.
Before you come to the office
If you are a new patient and you schedule your appointment in advance,
you should receive in the mail some paperwork, which you should complete
to the best of your ability before your appointment. This paperwork includes
a questionnaire about your health, medications, and past medical problems.
It also includes a registration form and a privacy law form. This information
is essential for us to give you the best possible medical care. All of
your personal medical information is held in the strictest of confidence.
If you have health insurance, your insurance plan may require a referral
from your primary care provider before you can be seen at our office.
When you call for your appointment, our staff should be able to tell you
if you need a referral or not.
We do file insurance for patients with many different health insurance
plans. Our staff can tell you if your plan is one in which we participate.
We do also see patients without health insurance. Payment for all out-of-pocket
expenses including co-payments, deductibles, and other non-covered service
is due at the time service is rendered.
If you are having a procedure done in the office, please remember to eat
something and take your regular medicines before your appointment.
When you arrive
Please arrive promptly at the time of your scheduled appointment and sign
in at the reception area.
If you are a new patient, or if you have not been to our office in over
three years, or if you have had significant changes in your health since
your last visit, please arrive 10 minutes earlier than your appointment
time so you can fill out or update the health questionnaire.
If you are unable to come to your scheduled appointment
Please call us as soon as you know you are not going to be able to make
it to your appointment or if you no longer need your appointment. We appreciate
at least 24 hours notice, so that we may better serve those patients who
need urgent appointments.
What to bring with you
New patients should bring your completed health questionnaire, personal
data sheet, and HIPAA privacy agreement if you received them in the mail.
Please do not mail or fax these back to us. Also be sure to bring your
health insurance card. If you are not the policy holder, we will need
the name and social security number of the policy holder as well.
If you are a return patient, please bring an updated list of your medications
with you.
